Further to the continued success of the inaugural Diabetes Mellitus Transformation and Sustainability Network series in 2017, we are delighted to announce they will continue into 2018, with a second East of England meeting planned.
The network aims to support the commissioning of high quality, outcomes focused and value for money diabetes services that support your local STP delivery.
The conference will be chaired by Professor Ian Macdonald – Professor of Metabolic Physiology, University of Nottingham on Wednesday 28th March, with a focus on:
- Transforming Prevention of Diabetes
- Transforming Outcomes for People with T2D
The meeting will bring together like minded people from primary, community, secondary care, and associated local authority, health, care and third sector organisations involved in the commissioning and delivery of services to care for and support patients with diabetes. This provides a platform for active discussion and exchange of ideas to address the opportunities to deliver diabetes services in a more integrated way.
The meeting begins with a registration and a buffet lunch from 12.30, with the meeting proper starting at 13.15 and ending at 16.45.
